Forms and Information

Thank you for your interest in our facility as a place for your child to be while you are busy.  Below are forms and other relevant information that we will require you finishing before your child can attend.  If you need help with any of the questions please come by or call us at (580) 223-7111 and we would be happy to help you.

Enrollment Packet

This packet is required of all new children.  Please print the forms and mail/bring them to us so that we may get them processed.  If you are unable to print it we will be happy to provide you with a copy.

Please be sure to have your legal documents, enrollment fee and tution ready with all forms filled out.

Rates as of July 1, 2017

Tuition Reductions: 10% for each additional child in the family.

Tuition Rates   18mo to 2yrs
5 days a week = $490 per month

Tuition Rates   2yrs to 4+yrs
5 days a week=440.00 per month

Enrollment Fee is $25 due at time of enrollment and at the beginning of each school year.

All tution is due at the first of each month in full.  We accept check, money order, cash and credit cards.

Health, Immunizations, Illness and Medication

Your child must be in good health attending school. We cannot keep your child with a rash, fever, or illness of any kind. If your child becomes ill or has accident at school, you will be contacted and he/she must be picked up within one hour. In the event of illness, a child may not return to school until symptom free for 24 hours without medication. Exceptions will be made only for those children who have been on antibiotics for 48 hours, are fever free and have a note from their doctor stating that they  are no longer contagious. The director has the discretion regarding illness and re-admittance to the school.

In case of extreme emergency, if the family cannot be reached, a doctor will be contacted and the child will be taken to the ER for treatment. The school cannot assume financial responsibility for emergency services rendered, including, but not limited to fees for ambulance, hospital or doctor.

Teachers are not allowed to give medication. All medicines must be signed in at the office. This includes prescriptions and non-prescription medication. Creams, ointments and even sunscreen also must be signed in. if your child needs a rescue inhaler or epi pen, they too need to be signed in at the office.

All required immunizations must be up to date.

HCCC is required by the state of Oklahoma to report to the department of human services any child showing signs of abuse or neglect.
